Embedding your Questionnaire

Within each Project or Package there is an area to embed a snippet of code which will bring in questionnaires you have created with the help of a 3rd party program. You can add multiple questionnaires to any Project or Package and can also create questionnaire templates!

Tip: No matter what you are using to create your questionnaire, as long as it can output an iFrame embed code then you should be able to embed it into Mydoma Studio.

You can also embed the code directly in the project or package questionnaire:

Click "Open questionnaire" to see and to fill out the questionnaire

Once your client has submitted the questionnaire to you, click the three vertical dots at the top right of the questionnaire

Then select "Complete" to move it over to the Completed Questionnaires.

Didn't mean to move it to completed or need your client to fill out the questionnaire again? Click the three vertical dots on the completed questionnaire

Then click "Uncomplete" to move it back over to Open Questionnaires.

Creating a Questionnaire Template

Once you've added your questionnaire to a Project or Package, if it's one you use regularly, turn it into a template!

Click the three vertical dots in the upper right-hand corner of the questionnaire you'd like to create into a template.

Then click "Create Template."

Give your template and name and description then hit "Save."

How to use a Questionnaire Template

Open a project or package and go to the Questionnaire tab. Click "Add Questionnaire."

Click on the Templates dropdown box to select the template you wish to use then click "Save."

Deleting a Questionnaire Template

To delete a Questionnaire Template, go to your Studio Settings. You will find the Studio Settings as the first menu in the the gear icon at the top of the left menu.

Click "Templates"

Click on the three vertical dots next to the template you'd like and click "Delete"

**Note** Deleted templates cannot be retrieved. You'll have to re-enter the template manually and save it again.

Where are my Questionnaire Responses?

In Mydoma, you are able to embed a 3rd party form as a template and reuse these for all of your projects and packages. Essentially, the embedded form is a shortcut to the form in the software you used to create it (Google Form, JotForm, etc.).

When the client responds and submits the form, the responses don't store in your Mydoma portal. Instead, they store in the software you used to create it.

That means you'll need to go to the software where you created your actual form/questionnaire to look for the responses.

To simplify the process of embedding your questionnaire, we have listed some popular forms that will allow you to export the snippet of code needed as well as how to download their submissions.

Google Forms

Embed

To get your embed code from Google Forms click on the "Send" button in the top right. In the popup that comes up click on the tab with the "<>" icon, then choose the sizing you want and copy the text.

Responses

Go to your Google Drive and open up the Google Form you'd like to download the responses from.

Click "Responses"

Click "Individual" and find the response you're looking for by using the arrows or typing into the first number box

Click the three vertical dots in the upper right-hand corner

Click "Print"

Change Destination to "Save as PDF" then click "Save."

Once it's saved onto your computer, you can upload the PDF like normal into your Project's Files & Media in your Studio.

JotForm

Embed

Once you are done creating your questionnaire in JotForm click on the publish button. In the publish menu select the "Embed" tab and then in the "Embed Options" dropdown select iFrame. Then just select the code and copy it or click on the "Copy code" button.

Responses

Login into your JotForm account and click "Submissions" on the questionnaire your client just filled out.

**Note** it'll show a number icon on the questionnaire with new submissions.

Select the submission you'd like to download then click the three vertical dots

Then go to "Download," "Download as PDF," "PDF Document."
